Sports Physicals: If your school will be doing after-school sports this school year, he or she must have a sport physical on record at the school. This physical is good for 12 months from the date of the physical.
Immunizations: Your student must be up-to-date on immunizations. This is not a whim of the school. It is mandated by the State of Washington. Be sure your student has received the second MMR. Most students received the latest Tetanus booster at age 4. It is due every 10 years. If your student is at least 14 years old, he or she is due for another tetanus booster. Evergreen Hospital will conduct a free Immunization Clinic for school-age children on Saturday, September 9, 2000, in the Evergreen Hospital Auditorium. An appointment is required. Parents must accompany student. Phone 425-899-3000 for an appointment.
Medication: If your student will be taking medication at school, there must be an authorization signed by the doctor and parent on file with the school nurse.
Health Screenings: The state-mandated health screenings will be conducted on Friday, September 22. 7th graders will be screened for vision, hearing, and scoliosis. 9th graders will will be screened for scoliosis. Be sure your student is in school this day. Parent volunteers are needed for these screenings. Please volunteer. Phone the school nurse at 402-5406 and leave a message if you can help wityh this one-day screening.
